Steps to be followed:

1. Add "/general-request" on your School URL.

2. After redirected to the page, fill up mandatory fields.

Date Requested | Full Name | Contact Number | Email | Grade/Year Level 

3. Scroll down to add items you wish to request.

4. Click the drop down arrow to choose what item you want to get. You can also request multiple items by click the Add New Item button. Also, input purpose of request.

Once done, click Save.
